Located in Brooklyn, NY
*Salary: $90K to $105K per year (dependind on experience)*
Description of the Case Manager:
- Assess new patients
- Update and revise patient health care plans as needed
- Educate patients and families about health care plans and options
- Make clinical decisions for routine patient care
- Accurately document and submit medical records
- Provide emotional support to patients and families
Qualifications of the Case Manager:
- Must have a valid New York RN license
- Certification in UAS preferred
- Must have prior experience as a Case Manager
- Must have experience in Assisted Living
- Strong written and verbal communication skills
