Position Overview:

As a Full-Time Hospice RN Case Manager, you will be a critical member of our care team, responsible for delivering high-quality, patient-centered care. You will manage a caseload of hospice patients, ensuring their physical, emotional, and spiritual needs are met during the end-of-life process. Your role involves collaboration with interdisciplinary teams, patients, and their families to develop and implement individualized care plans.

Key Responsibilities:

• Manage and coordinate care for hospice patients, providing direct nursing care based on each patient’s individual needs.
• Conduct regular patient visits to assess physical, emotional, and spiritual needs, adjusting care plans as necessary.
• Collaborate with interdisciplinary team members, including physicians, social workers, chaplains, and home health aides, to provide comprehensive care.
• Educate patients and families on hospice care, symptom management, and the dying process.
• Ensure timely documentation and compliance with all regulatory requirements.
• Provide on-call coverage as part of the rotation to support patients and families outside of regular business hours.
• Serve as a liaison between patients, families, and the healthcare team to ensure clear communication and continuity of care.
• Maintain accurate and timely documentation in electronic health records.

Qualifications:

• Active RN license in the state of California.
• Minimum of 2 years of experience as an RN, with at least 2 year in hospice or palliative care.
• Strong understanding of hospice philosophy and standards of care.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of an interdisciplinary team.
• Compassionate, patient-centered approach to care.
• Valid driver’s license and reliable transportation.
