Monday, Wednesday, Thursday, Friday: 6:30am -8:30 am

Monday: 3:30pm-7:30pm

Saturday: 7:00am -1:00 pm

Requirements:

Background check clearance

Motor Vehicle Record check clearance

Must have a valid Louisiana Driver’s License

Must have reliable vehicle with Auto Insurance and Louisiana Registration

Duties:

Follow the client’s plan of care and implement client’s goals

Report and document all incidents immediately and refusal of care

Complete accurate and timely progress notes/time-sheets

Clock in/out of the Electronic Verification System

Maintain client’s confidentiality and rights at all times

Maintain the client’s home to ensure it is clean, safe, and comfortable

Assist client with maintaining their health and safety

Promote/Assist/Teach the client personal hygiene tasks

Assist/Monitor/Instruct client with: Meal Preparation, Food Storage, and Clean-up; Grooming/bathing; Housekeeping; Laundry; Toileting

Medication/Appointment Reminders

Transport Client to/from medical appointments/ grocery shopping/community outings

Provide support with client’s hobbies, social activities, and community outings
