AMN Healthcare Allied is seeking a travel Physical Therapist for a travel job in TUBA CITY, Arizona.
Job Description & Requirements
StartDate: 7/29/2024 Pay Rate: $1650.00 - $2100.00
PT- Rehab needed ASAP ORIENTATION: Every two (2) weeks only. COVID EXEMPTIONS: We do not accept religious exemptions. If a candidate submits for a medical exemption, we will have to get approval from our medical review board to proceed with onboarding. ** Fingerprinting must be submitted and cleared prior to start. ** REQUIRED: Bachelor’s degree from the Commission of Accreditation in Physical Therapy Education (CAPTE) accredited school of Physical Therapy. Must possess a valid, current, full and unrestricted license in any state within the United States, District of Columbia, or the Commonwealth of Puerto Rico during tenure of employment. Must have and maintain a current Basic Life Support (BLS) certification by the American Heart Association. Applicant must meet requirements to be credentialed to serve on the Medical Staff. All related information required for credentialing must be provided. HOUSING APPLICATION: must be submitted 2 weeks prior to start date - form uploaded in Medefis
