We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers.
The part-time Optometrist sees Amoskeag Health primary care patients referred by their Amoskeag Health primary care providers for consultation on a variety of eye-related issues including but not limited to diabetic eye evaluations, complaints of decreased visual field, complaints of decreased visual acuity, eye pain, eye discharge, eye infections, management of glaucoma etc.
Position requires 8 hours/week on-site of patient care time at Amoskeag Health filling in when the Medical Director of the Eye Care Center is not available for clinical care.
