Job Description

Job Title: Home Health LPN

Job Summary:

We are seeking a compassionate and dedicated Home Health LPN to provide high-quality care to patients in the comfort of their own homes. The ideal candidate will have excellent clinical skills, attention to detail, and a strong commitment to patient well-being.

Responsibilities and Duties:

• Conduct in-home assessments and develop care plans in collaboration with the supervising RN or physician
• Administer medications, perform wound care, and provide other skilled nursing services as needed
• Monitor patients' vital signs and condition, and communicate any changes to the healthcare team
• Educate patients and their families on self-care techniques and medication management
• Coordinate with other healthcare providers to ensure continuity of care
• Maintain accurate and thorough documentation of all patient interactions and care provided

Qualifications and Skills:

• Current LPN license in the state of practice
• Minimum of 1 year of experience in a clinical setting, preferably in home health
• CPR certification
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work independently and prioritize tasks effectively
• Valid driver's license and reliable transportation
