Job Title: LPN, Long Term Care (LTC)

Job Summary:

As an LPN in Long Term Care (LTC), you will be responsible for providing quality nursing care to residents in long-term care facilities. Your primary role will be to assess patients' needs, develop care plans, administer medication, and collaborate with other healthcare professionals to ensure the well-being of residents.

Responsibilities and Duties:

• Conducting patient assessments and developing individualized care plans
• Administering medication and treatments as prescribed by healthcare providers
• Monitoring patients' overall health and well-being
• Collaborating with physicians, nurses, and other healthcare professionals to ensure comprehensive care
• Documenting patient progress and communicating with family members and caregivers
• Providing emotional support and assistance to residents and their families
• Implementing infection control protocols and ensuring a safe environment for residents
• Assisting with activities of daily living, such as bathing, dressing, and eating

Qualifications and Skills:

• Current LPN license in the state of practice
• Previous experience in long-term care settings is preferred
• Strong communication and interpersonal skills
• Ability to work effectively in a team environment
• Proficiency in basic nursing skills and medical terminology
• Compassionate and patient-centered approach to care
