Responsibilities :

• Provide personal care and assistance to hospice patients in their homes or healthcare facilities
• Assist with activities of daily living, including bathing, dressing, grooming, and toileting
• Monitor and document patient vital signs and report any changes to the nurse or supervisor
• Assist with tube feeding and catheter care as needed
• Prepare meals and assist with feeding if necessary
• Provide companionship and emotional support to patients and their families
• Maintain a clean and safe environment for patients

Qualifications :

• Expereince documenting visit notes on a software platform
• Previous experience in hospice care or a related field is preferred
• Knowledge of medical terminology is a plus
• Ability to lift and transfer patients safely
• Compassionate and empathetic nature
• Excellent communication skills, both verbal and written
• Ability to work well with individuals of all ages, including those with developmental disabilities
• Strong organizational skills and attention to detail
