Responsibilities:
- Provide personal care and assistance to hospice patients in their homes or healthcare facilities
- Assist with activities of daily living, including bathing, dressing, grooming, and toileting
- Monitor and document patient vital signs and report any changes to the nurse or supervisor
- Assist with tube feeding and catheter care as needed
- Prepare meals and assist with feeding if necessary
- Provide companionship and emotional support to patients and their families
- Maintain a clean and safe environment for patients
Qualifications:
- Minimum 2 yrs expereince as a CNA with an active IN license
- Expereince documenting visit notes on a software platform
- Previous experience in hospice care or a related field is preferred
- Knowledge of medical terminology is a plus
- Ability to lift and transfer patients safely
- Compassionate and empathetic nature
- Excellent communication skills, both verbal and written
- Ability to work well with individuals of all ages, including those with developmental disabilities
- Strong organizational skills and attention to detail
