I. SUMMARY OF POSITION PURPOSE

Provides personal care and incidental household services to assigned patients in their place of residence under the direction and supervision of a Home Health RN or Therapist.

II. SPECIFIC DUTIES AND RESPONSIBILITIES

• Performs personal care and incidental household services for each assigned patient according to the written directions completed by the RN or Therapist.
• Reports appropriate concerns to RN or Manager
• Maintains current clinical record documentation regarding services provided to each patient.
• Utilizes effective communication skills to promote excellent customer satisfaction, patient care, and unit operations.
• Maintains expected productivity standards.
• Participates in home health agency functions as required by agency policies as well as accrediting and regulatory organizations.
• Identifies learning needs and assumes responsibility for seeking out opportunities to meet general and unit specific competencies.
• Other duties as assigned.

III. EDUCATION/QUALIFICATIONS

Minimum Educational Level Required:

• High school diploma or GED certificate preferred
• Must have ability to read, write and execute instructions.

Length and Type of Previous Job Experience Required:

• Previous nursing assistance experience preferred.

Professional Certifications Required:

• Nursing Assistant Certificate required.

Other Special Training/Competencies Required:

• Home health aide training and competency evaluation or competency evaluation is required.
• Must successfully complete the agency’s competency evaluation.
• Must be capable of assisting with activities of daily living and following a care plan with minimal supervision.
• The ability to provide reliable transportation as required by the nature of the job.
