Responsibilities:
- Determine patient medical necessity
- Conducting care coordination and educating patients on their condition, treatment options and benefits
- Develop, monitor and review treatment plans
- Ensure that all incoming consults are triaged within two business days of receipt
- Contact patients with negative results within three business days of when they are returned
- Update patient tracker with relevant patient information with minimal errors
- Ensure that consults to any other service are input and read for signature
- Ability to successfully undergo a Government sponsored background investigation (US Citizenship Required)
- Associate Degree or Bachelor of Science in Nursing from an accredited program, college, or university and hold a current license to practice as a Registered Nurse in any State; a Bachelor’s degree is preferred
- Experience with RN activities in providing hematology/oncology care for a minimum of 2 years
- Telehealth Care Coordination/Case Manager experience required
- Specialization in Genetics
- Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) Certified
- Familiarity with medical terminology, hospital terminology and/or clinics
- Technically proficient in the skills necessary to fulfill the government's requirements; will be provided training by the VA on appointment management, VA policy and procedures
- Sit, stand, walk, lift, squat, bend, twist, and reach above shoulders during the work shift
- Lift up to 50 lbs from floor to waist
- Lift up to 20 lbs
- Carry up to 40 lbs a reasonable distance
- Push/pull with 30 lbs of force
