Description

The health services that became theAnthony L Jordan Health Corporation (http://www.jordanhealth.org/) (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants ofRochester, NY (http://www.city-data.com/city/Rochester-New-York.html) and in Canandaigua, N.Y.

Jordan Health is now seeking aRetention Specialistwho, under the guidance of the Director of Quality Assurance, is primarily responsible for providing action and ideas in the development of strategies to increase unique active patients.

Requirements

TheRetention Specialistwill ensure to:

Create systems that reach out to roster patients from our payer groups as well as patients whom have not accessed care at Jordan for greater than 12 months.

Creates work flows, analyzes data, and communicates with payers around rosters of Jordan patients.

Facilitates the organizations outreach to new patients to reduce barriers to first appointment, improves in-reach to patients whom have not been to a provider visit in greater than 12 months.

Collaborates with Quality team members and practice managers to create and implement tools, processes, and systems to optimize clinical quality, patient safety and care management.

Education And Experience Required:

Associate degree or working on the degree.

Combination of education and relevant work experience will be considered.

High level of work knowledge and understanding of the Microsoft Office suite of software, specifically Excel, Word, and PowerPoint. Knowledge of Access a plus.

High level of work knowledge and understanding of eClinical Works or other electronic health record EHR.

Experience with other electronic reporting tools such as BridgetIT, Phytel, i2i.

Healthcare background, experience and performance that promotes a high level of credibility with clinical professionals.

Licenses And Certifications:

None Required.

Special Skills, Knowledge Required:

Excellent written, oral, and interpersonal skills. Highly self-motivated, directed and change oriented.

Good analytical, problem-solving skills and ability to effectively prioritize and execute task under pressure.

Able to work, contribute to flourish in a team centric, collaborative environment.
