Overview:
Position Summary: Provides vital assistance and support to Disease Control staff, including nurses, investigators, managers, and other Community Health Workers (CHWs), in disease investigation and outbreak management. This role involves community member interviews, appointment scheduling and follow-ups, and locating clients with updated addresses or phone numbers. Responsibilities include conducting client interviews, assisting with health and social history forms, checking immunization status, contacting clients for information gathering and follow-up, and maintaining client confidentiality.
Key Responsibilities:

• Client Interaction and Support:

o Conduct interviews and assist clients in completing health and social history forms. o Prepare and maintain client case files, logs, and records. o Promote health, safety, and disease prevention in the community. o Perform related duties as assigned by supervisors.

• Schedule and follow up on appointments, and locate clients with changed contact information.
• Contact clients to gather information and provide follow-up documents, ensuring confidentiality.
• Administrative Duties:
• Print and distribute food and service vouchers as directed.
• Complete tally sheets, questionnaires, and duplicate necessary forms and records.
• Assist with data entry for disease tracking.
• Community Outreach and Education:
• Connect clients to health care services, considering their linguistic and cultural needs.
• Provide basic health information on preventive care and disease control.
• Act as an interpreter and translator for non-English speaking clients, if qualified.
• Maintain and distribute educational materials.
