Job Description : We areseeking a dedicated Early Intervention Service Coordinator to joinour team. The Service Coordinator will manage services for childrenenrolled in the HOPE Program ensuring they receive comprehensivesupport.

This role involves processing referrals maintainingrecords coordinating services and working closely with families todevelop Individualized Family Service Plans (IFSPs).

KeyResponsibilities :

• ServiceCoordination : Serve as a service coordinator toassigned children and theirfamilies.
• ReferralProcessing : Process new referrals and maintain childrecords.
• Documentation : Document case management / service coordination provided and anyother information pertinent to the childs wellbeing andcase.
• ServiceCoordination : Coordinate services for childrenenrolled in the HOPEProgram.
• ReviewMeetings : Conduct 6month and annual reviews as wellas transition meetings.
• IFSPDevelopment : Complete Individualized Family ServicePlans (IFSPs) withfamilies.
• Evaluation andAssessment : Evaluate assess and maintain programparticipant records.
• EligibilityEvaluation : Evaluate pending and annual referrals todetermine eligibility.
• AdditionalDuties : Perform other related duties asassigned.

Qualifications :

• Education : Bachelors degree which meets AEIS Professional Standardsrequired.
• Knowledge : Working knowledge of typical childdevelopment.
• Skills : Proficiency in Microsoft Word and Excel excellent written andverbal communicationskills.
• Organization : Ability to prioritize effectively and maintain efficientorganization.
• Bilingual (Fluent Spanish andEnglish)

PhysicalRequirements :

• Must beable to physically maneuver with small children in positions thatinclude sitting standing kneeling crouching and crawling attimes.
• Occasionally required tosit.
• Specific vision abilities required includeclose vision distance vision color vision and depthperception.
