Job Description: Long Term Care Consultant (LTCC) Assessor

Location: 2155 Arlington Ave., OH, 43609

Job Summary

The Long Term Care Consultant (LTCC) Assessor will be responsible for assessing the needs of individuals seeking long-term care services and providing guidance on available options. The LTCC Assessor will work closely with clients and their families to develop care plans that meet their unique needs and preferences. The successful candidate will be required to secure and uphold certification from the Ohio Department of Aging as well as certification from Ohio Department of Insurance for the Ohio Senior Health Insurance Information Program (OSHIIP).

Key Responsibilities

  • Conduct assessments of individuals seeking long-term care services
  • Develop care plans that meet the unique needs and preferences of clients
  • Provide guidance on available long-term care options
  • Work closely with clients and their families to ensure their needs are met
  • Maintain confidentiality and follow established AOoA confidentiality HIPAA procedures
  • Stay up-to-date on changes in long-term care regulations and policies
  • Collaborate with other healthcare professionals to ensure the best possible care for clients

Requirements

  • Certification from the Ohio Department of Aging
  • Certification from Ohio Department of Insurance for the Ohio Senior Health Insurance Information Program (OSHIIP)
  • Excellent communication and interpersonal skills
  • Strong organizational and time-management skills
  • Ability to work independently and as part of a team
  • Knowledge of long-term care regulations and policies
  • Ability to maintain confidentiality and follow established AOoA confidentiality HIPAA procedures
