We are a support services organization committed to making a positive impact in the communities we serve and are actively seeking a dedicated and empathetic Case Manager to focus on assisting the reentry population. This position will work out of the AAY Seattle office, is a part-time position, and reports to the Program Director.

Duties:

• Identifies, locates, and determines the status of justice-involved participants.
• Advocates on behalf of and facilitates coordination of resources required to help justice-involved participants reach optimum functional levels upon reentry to the community during transition and provides needed case management.
• Works with the WA State Prison, county jails, DOC, and a network of reentry organizations to connect with providers, advocates, contracted vendors, and community resources to assess the member's status, identify needs and ensure access to appropriate services to achieve positive outcomes upon member’s reentry to the community.
• Performs the primary functions of assessment, planning, facilitation, and advocacy through collaboration with the member, county jail, work release or DOC, and other resources involved in the member's reentry.
• Meets with justice-involved participants in person or via phone at the office whether by walk-ins or by appointments.
• Provide transportation passes to participants for appointments (legal, medical) if needed.
• Attend all staff meetings and training, as required by supervisors.
• Connects with participants to achieve their outlined goals.
• Have the ability to work with an ever-growing and evolving caseload.
• Clearly document actions and interactions with participants on AAY’s data software.
• Maintains healthy boundaries with participants while serving high-need individuals in reentry who may also be experiencing crisis and poverty.
• Preliminary screening with current participants of the community to assess needs
• (housing, employment, education, mental & spiritual health, substance abuse treatment, legal obligations, overall health.)
• Assist participants in creating goals or an action plan & continually follow up.
• Coordinate with other case managers in the network for resources and contacts.
• Daily debriefs with the Re-entry Coordinator to inform any changes to participant's reentry goals is mandatory.
• Create and report monthly data/statistics to the Re-entry Coordinator.
• Performs other duties as assigned.

Administrative

• Maintain client files and add new reentry goals set forth by the participants when provided.
• Assists participants fill out paperwork and attend to daily correspondence
• Relay participant concerns to the Re-entry Coordinator

Physical Requirement

While performing the duties of this job, the employee is regularly required to: work at a stationary workspace for long periods of time, with mobility to move throughout the office; use a PC, constantly reaching with hands and fingers and keyboarding extensively; work with team as frequent communication is required, including use of the telephone; be able to read and write documents in hard and electronic copy, including those requiring great attention to detail; have the physical strength and agility to handle routine office tasks and machinery.

Skills/Qualifications Required

• One year working in high-risk/at-risk populations but any experience with at-risk/vulnerable populations is a plus.
• Ability to converse with and listen to people in many different settings.
• Knowledge and experience with Microsoft software: Excel, Word.
• Ability to use a computer and telephone as well as other standard office equipment.
• Ability to be trained in data software and have high communication skills.
• One-year case management or closely related position experience.
• Lived experience within the justice system and /or experience with justice-involved
• individuals.
• The noise level is moderate. Frequent interruptions are common, while under deadlines and time constraints.

Job Type: Part-time

Pay: $23.00 - $26.75 per hour

Expected hours: 20 per week
