Arundel Lodge, Inc. is a non profit organization located in Edgewater, MD that works toward a world where behavioral health disorders are not a limit to achieving a meaningful life in the community. The Health Home Case Manager will be a liaison with the client, family members, healthcare providers, and insurance companies. Some of the tasks include;

• Advocating and coordinating for the psychiatric and somatic care of our clients. Work with pharmacies to obtain medications, supplies, and durable medical equipment.
• Coordinate care with physicians, hospitals, pharmacies, nursing facilities.
• Assist with finding providers and specialists that accept client's MCO or Medicare.
• Coordination of residential hospital admissions and discharges.
• Scheduling labs and toxicology screening.
• Assisting prescribers and RN with obtaining pre-authorizations.
• Assist with residential hospital admissions and discharges.

Education; BS/BA/AA in Health, Human Services, or Certified Nursing Assistant.

Experience; One year experience working in Human Services is preferred, a Certified Nursing Assistant is a plus.
