Description
Intensive Case Management (ICM) provides mental health treatment services and supports that help individuals with severe mental illness achieve their recovery goals.
ICM services assist individuals in minimizing the negative effects of symptoms of mental health problems and addictive diseases that interfere with daily living, independent functioning and personal development.
The ICM case manager works with the individual and other support and service providers to develop an Individual Recovery Plan (IRP) that includes the medical, behavioral, social, educational, vocational, housing, financial and other needs of the eligible individual.
Services help individuals develop strategies and support to avoid out-of- home placement and reduce the need for more intensive services, and coordinate access to other needed community services and resources.

NOTE: A pre-employment drug screen, random drug screen, and state & federal criminal background investigation will be required of selected applicant.
Effective July 1, 2017 smoking or use of any tobacco products are prohibited on company premises.
