***This position can have a flexible, remote/hybrid schedule but should be available on an as needed basis to conduct intakes with new families and annual in person assessments. Monthly hour’s commitment: ~20 hours***

Responsibilities:

• Conduct intakes, annual assessments, and periodic health screenings
•  Provide training and consultation to programs regarding medical issues, procedures and techniques, and health and wellness practices.

• Provide clinical consultation and crisis intervention as needed.

• Provide guidance, medical oversight, and individualized consultation to people served, families, and staff to teams to ensure overall health and well-being of people served.
• Assess and provide individualized education related to person’s health and disease processes; reinforce wellness and preventative health measures.
• Review clinical records on a regular basis to ensure adequate services and supports are provided, including any applicable medical interventions, tests, or preventative care as necessary.
• At as a liaison between the individual’s family/care providers and medical providers to ensure timely and appropriate treatment.

• Attend facilitate, and/or participate in 1:1 and group meetings and trainings as assigned.

• Perform other duties as assigned.

Qualifications

• Registered Nurse licensed in Massachusetts
• Previous experience with adult/youth with intellectual disabilities, autism spectrum disorder, or psychiatric disabilities
• Excellent assessment, documentation, and communication skills required.
• Valid driver’s license and acceptable driving record
