Duties :
- Makes home visits to adults to assess their needs.
- Interviews, makes initial assessments & referrals.
- Manages cases of social service recipients.
- Investigates & verifies information obtained. Provides health related and social services to clients.
Skills :
- Discharge Planning experience is required.

Education : Bachelor's Degree
Schedule Notes :
- Hospital experience is required
- Discharge Planning experience is a must
- Extensions are not guaranteed
