We are seeking a dynamic and experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our client/s team. In this role, you will have the opportunity to provide high-quality healthcare services to patients through telehealth platforms while working onsite at their facility. The Telehealth NP/PA will play a crucial role in expanding access to care, promoting continuity of care, and delivering patient-centered services.

Key Responsibilities:

• Conduct virtual patient encounters through telehealth platforms, including video visits and telephonic consultations, to assess and address patients' healthcare needs.
• Perform comprehensive health assessments, diagnose medical conditions, and develop individualized treatment plans in collaboration with patients and other members of the healthcare team.
• Prescribe medications, order diagnostic tests, and make referrals to specialists as needed to ensure optimal patient care and management.
• Provide patient education on preventive health measures, chronic disease management, medication adherence, and lifestyle modifications to promote overall wellness.
• Document patient encounters accurately and timely in the electronic health record (EHR) system, ensuring compliance with regulatory standards and organizational policies.
• Collaborate with onsite clinicians, nurses, and support staff to coordinate patient care and facilitate seamless transitions between virtual and in-person visits.
• Participate in quality improvement initiatives and ongoing professional development activities to enhance clinical knowledge and skills in telehealth practice.

Requirements:

• Master's degree in Nursing (NP) or Physician Assistant Studies (PA) from an accredited program.
• Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in NY.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), or Physician Assistant (PA-C) preferred.
• Minimum of 1 year of clinical experience as an NP or PA, with proficiency in primary care and telehealth practice.
• Strong clinical assessment and diagnostic skills, with the ability to formulate evidence-based treatment plans.
• Excellent communication and interpersonal skills, with the ability to establish rapport and build trust with patients in a virtual setting.
• Comfortable using telehealth platforms and technology for remote patient care delivery.

Please note- The annual base salary range is $80k to $100k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
