Our client is a leading mental health agency committed to providing trauma-informed care to individuals in need of support and they are seeking a Trauma Therapy Case Manager to join their team.

Responsibilities:

• Conduct thorough assessments to understand the trauma history, emotional well-being, and psychosocial needs of clients.
• Collaborate with other professionals and agencies to gather comprehensive information for a holistic understanding of the client's situation.
• Develop and implement individualized trauma-informed case plans in collaboration with clients, ensuring goals are realistic, measurable, and aligned with their unique needs.
• Regularly review and update case plans based on the client's progress, setbacks, and changing circumstances.
• Offer trauma-focused counseling services using evidence-based therapeutic approaches.
• Collaborate with multidisciplinary teams, including mental health professionals, medical practitioners, educators, and community organizations, to provide comprehensive care.
• Maintain accurate and confidential client records, documenting assessments, case plans, interventions, and progress.

Requirements:

• MSW, MHC, LMSW, or LMHC license in New York
• At least 1 year of experience in case management and counseling
• Bilingual preferred

Please note- The annual base salary range is $55k to $70k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
