Physical Therapist | Home Visits, Grand Rapids Area| $5K Sign On Bonus | Full Time 

Join our outstanding team of home health professionals as a Physical Therapist in the Grand Rapids Area with a $5,000 Sign-On Bonus!Atrio Home Care, a partnership between Holland Home, Clark Retirement, and Resthaven, is looking for a compassionate Physical Therapist to join our Grand Rapids team. This position involves providing physical therapy services in the home setting to clients within a 35-mile radius of Grand Rapids.

Responsibilities Physical Therapist | Home Visits, Grand Rapids Area| $5K Sign On Bonus | Full Time 

• Conduct initial physical therapy evaluations and ongoing assessments to determine functional level
• Develop treatment plan for clients in consultation with the physician and other Atrio Home Health staff
• Evaluates equipment needs to increase functional level and coordinates acquisition of needed durable medical equipment
• Provide physical therapy treatment including gait training, transfer training, balance training, conditioning, and extremity strengthening
• Identify barriers to desired outcomes and evaluates outcomes of care
• Prepare clinical documentation, progress notes, and other required documentation
• Participates in discharge planning with the Atrio Home Health team
• Deliver clinical education to students
• Address and resolve client and family concerns related to therapy

Location: Grand Rapids, covering a 35-mile radius around GR

Shift: Day shift 8-5pm, with flexibility in scheduling to help maintain a healthy work/life balance

Weekend On Call Rotation: approximately once every two months

Compensation

• Our compensation model rewards our Physical Therapist for the work they do and offers bonuses to them for high-quality client outcomes, productivity, and efficient case management. 
• Premium pay weekend and holiday visits

Sign On Bonus

• $5,000 bonus; paid in 3 increments over the course of your 1st year of employment

Qualifications

• Physical Therapist License in the State of Michigan
• Minimum of one year experience as a Physical Therapist; prior certified home health experience preferred
• Geriatric experience is highly valued
• Valid Michigan driver's license, auto insurance, and reliable means of transportation for home visits
