Attentive Care Service Agency is Hiring a HOME CARE FIELD COORDINATOR Near Philadelphia, PA

JOB SUMMARY

Home Care Field Coordinator is responsible for greeting incoming clients, recording their personal
details, gathering medical histories as applicable, and obtaining any health insurance
information they have as applicable. A Field Coordinator is highly organized and detail-oriented,
excellent interpersonal skills, with a strong aptitude for great customer service. The duties
for an Intake Coordinator include, but are not limited to, the following:

Responsibilities

  • Visiting the home of new clients and processing Agency paperwork for admission to the Agency
  • Monitor the care performed by the caregiver during monthly supervised visits
  •  Provide weekly updated issues, changes, and concerns pertaining to clients and caregivers to the Administrator
  • Report all disciplinary issues concerning the caregivers to the HR Manager
  • Conduct monthly supervisory visits to clients to ensure quality of care
  • Participate in quarterly staff meetings
  • Visiting current clients as needed to update care plans, personal information, etc
  • Ensuring all client paperwork is completed and processed per agency protocol
  • Notifying applicable staff of new client start dates
  • Communicating with applicable staff with any client concerns
  • Ensuring new client visits are scheduled in a timely manner
  • Communicating with office staff
  • Answer phone and email inquiries for new client intakes
  • Comply with the regulations and policies of the Agency
  • Able to build a sustainable rapport with clients and employees

Qualifications

  • CNA certification with at least 2 years of HHA experience
  • Supervisor experience preferred
  • Must own an insured vehicle
  • Knowledge in the HHA field 
  • Flexible schedule
  • Strong multi-tasking and organizational skills
  • Professional references are strongly advised

Shifts: This is a hybrid/per diem position. 

Please note that a CNA certification is mandatory for this position, but experience entry level experience is accepted.
