A Social Worker with Ave Maria Hospice

Provides psychosocial support and other Social Work services to Hospice patients and their families in the patient's home, nursing home, assisted living, or other settings, in pre and post bereavement phases of Hospice service. Works to serve as social work member of hospice IDG and as liaison between hospice staff and nursing home staff. Develops social work plan of care, goals, and makes recommendations based on patient/family needs for assigned patients.

Job Requirements

Applicant must have a current license for Social Work. A valid drivers license, automobile insurance, and reliable transportation are required. Must be KIND, COMPASSIONATE, and HONEST. Self-driven with high personal integrity, good communication skills, observant, skilled in problem solving and prioritizing needs.

Experience

1 or more years experience in Hospice or Home Health preferred.
