We are seeking a highly skilled general cardiology advanced practice provider (APP) to join our dynamic cardiology clinic team. As an Advanced Practice Provider, you will play a pivotal role in providing independent comprehensive care and clinical expertise to patients across various cardiology subspecialties. You will have the opportunity to work with a diverse patient population and collaborate with a multidisciplinary team of healthcare professionals to deliver high-quality care.
Responsibilities:
- Provide direct patient care in the cardiology clinic, Avera Heart Hospital, Avera McKennan Hospital, and North Central Heart Outreach clinics evaluating and managing patients with a wide range of cardiac conditions, including but not limited to coronary artery disease, arrhythmias, heart failure, post cardiovascular surgical, valvular diseases, and hypertension.
- Conduct thorough patient assessments, including reviewing medical history, performing physical examinations, and ordering and interpreting diagnostic tests such as electrocardiograms, echocardiograms, and stress tests.
- Develop and implement individualized treatment plans based on patient’s conditions, needs, and preferences, incorporating evidence-based guidelines and the latest advancements in cardiology.
- Prescribe medications, perform medication reconciliations, and provide patient education on medication use, potential side effects, and interactions.
- Administer various cardiac procedures and treatments, including venipunctures, IV therapies, cardiac monitoring, and patient counseling on lifestyle modifications.
- Collaborate with cardiologists, nurses, and other healthcare providers to coordinate patient care, ensuring seamless transitions between outpatient and inpatient settings, as well as effective communication and continuity of care.
- Participate in shared on-call duties, responding to urgent or emergent cardiac cases and providing appropriate triage and interventions.
- Educate patients and their families about cardiac conditions, treatment options, self-care strategies, and preventive measures, empowering them to actively participate in their cardiac health management.
- Assist in cardiac procedures, such as stress testing, pacemaker checks, and device interrogations, under the supervision of cardiologists.
- Document patient encounters, assessments, treatments, and interventions accurately and timely in the electronic health record (EHR), ensuring compliance with regulatory and organizational guidelines.
- Engage in professional development activities, staying abreast of current cardiology research, emerging trends, and evidence-based practices.
