Avera is Hiring a Heart Failure Advanced Practice Provider Near Sioux Falls, SD

We are seeking a compassionate and experienced Heart Failure Advance Practice Provider to join our healthcare team. As a Heart Failure Advance Practice Provider, you will provide comprehensive care and management to patients diagnosed with heart failure. You will collaborate with physicians, nurses, and other healthcare professionals to deliver high-quality, patient-centered care throughout the continuum of heart failure treatment.
Responsibilities:
- Conduct thorough assessments and examinations of patients with heart failure, including reviewing medical history, performing physical examinations, and ordering and interpreting diagnostic tests.
- Develop and implement individualized treatment plans for patients with heart failure, incorporating evidence-based guidelines and considering patients' specific needs, comorbidities, and preferences.
- Collaborate with physicians and other healthcare providers to coordinate and optimize heart failure care, including medication management, lifestyle modifications, and follow-up appointments.
- Educate patients and their families about heart failure, its causes, symptoms, and management strategies, ensuring they have a clear understanding of the condition and the importance of adherence to treatment plans.
- Monitor patients' progress, evaluating treatment effectiveness, assessing for potential complications, and making necessary adjustments to the care plan.
- Perform medication reconciliations, prescribe medications, and provide medication counseling to promote medication adherence and optimize therapeutic outcomes.
- Provide counseling and support to patients and their families, addressing psychosocial concerns, lifestyle modifications, and coping strategies related to heart failure.
- Collaborate with the interdisciplinary team to ensure smooth transitions of care, including hospital admissions, discharges, and referrals to other healthcare providers as needed.
- Participate in quality improvement initiatives and research activities to enhance heart failure care and contribute to the advancement of knowledge in the field.
- Stay updated with the latest advancements in heart failure management, treatment guidelines, and pharmacotherapy, and incorporate evidence-based practices into clinical decision-making.
- Maintain accurate and timely documentation of patient encounters, assessments, treatments, and interventions in accordance with regulatory and organizational requirements.
Requirements:
- Current unrestricted Nurse Practitioner license in South Dakota.
- Master's degree in Nursing with a specialization in Adult-Gerontology or Family Nurse Practitioner.
- Certification as a Nurse Practitioner in Adult-Gerontology or Family Practice.
- Minimum 3 years of experience as a Nurse Practitioner, with a focus on managing patients with heart failure.
- In-depth knowledge of heart failure pathophysiology, treatment modalities, and current guidelines.
- Excellent clinical assessment and diagnostic skills, with the ability to make sound clinical judgments and decisions.
- Strong communication and interpersonal skills to effectively collaborate with patients, families, and interdisciplinary healthcare teams.
- Proficiency in using electronic health records (EHR) and other healthcare software applications.
- Dedication to providing patient-centered care, with a focus on empathy, compassion, and a holistic approach.
- Ability to work independently, prioritize tasks, and manage time effectively in a fast-paced clinical setting.
