Exceptional Home Care is Hiring a LVN - Licensed Vocational Nurse Near Long Beach, CA

We are seeking Licensed Vocational Nurses (LVN) to join our team! LVN's must have 1 year of LVN experience on their California license.

We have several Full, Part Time & Per Diem LVN shift care cases throughout the LA and OC areas. We are looking to hire LVN's to work 6-12 hour shifts. Flexible schedule. Must have experience with GT, Trach, and Vents to care for our sub-acute pediatric and adult patients doing one to one care in the home setting.

All shifts available 7-3pm, 3p-11pm, 11p-7am Sun - Sat.

Credentials:

  • Current LVN License, CPR, TB or Chest X-Ray, Physical, Auto Insurance, Driver License, Social Security Card, Covid Vaccine Card * (Or Declination Statement of Covid Vaccine)
  • Current Trach/Vent Certificate *(Valid within the last 3 yrs) If nurses do not have a current Trach/Vent Certificate * our agency will refer you to a 3rd party to get your certificate within 30 days of hire date.

Qualifications:

  • LVN's must have 1 year of LVN experience on their California license.
  • Some computer, tablet, or smart phone exp. preferred. Electronic medical record charting is done for daily documentation for the Skilled Nursing Flowsheet, Medication Profile, Physician Orders, Clock in/Clock Out.
  • Some knowledge of EMR (Electronic Medical Record) charting. We are currently using Synergy for our EMR.
  • Experience with G-Tubes, Trach, Vents, Oxygen.
  • Knowledge of proper head to toe assessment, and knowledge of proficient 2 hour nursing documentation to justify nursing services.
  • Previous experience in home care, home health nursing or other medical fields.
  • Familiarity with medical tools and equipment.
  • Ability to build rapport with patients.
  • Compassionate and caring demeanor.
  • Excellent written and verbal communication skills.

​Responsibilities:

  • Proper documentation of the following but not limited too; Vital Signs, Intake/Output, Seizure and Suction Log, head to toe assessment. Proper Wound Care measurements and staging, administration of breathing treatments, oxygen, medications, feedings. All EMR documentation must be turned in by the end of the shift.
  • Monitor for safety, signs/symptoms of respiratory distress, dehydration, seizures, & aspiration. Report changes in medical condition to PCG, RN, Director of Nursing, Physician.
  • ADL's, passive/active Range of Motion as needed, 2 hour reposition as tolerated. Range of motion should be done every day.
  • 2 person transfers or Hoyer Lift transfers for patients weighting more than 35 pounds.
  • It is mandatory to have patients faces cleaned/wiped; patients should have a bed bath, or tub bath everyday or every other day.
  • Patient should be left dry and prepared for the next shift.
  • Peri care is to be done every diaper change and prn.
  • Oral care is to be done every day, including shaving.
  • Make sure to keep your patient clean at all times.
  • Assist parent/guardian with patient while at medical, therapy, specialty appointments or family outings as needed.
  • Report and document any changes in regards to the patients medical condition, medications, feedings, etc to patient's medical team while at medical appointments either in person, via zoom, phone call, etc. and relay information to agency's RN's, Director of Nursing, and office staff as needed to ensure continuity of care.
  • Follow agencies policies, procedures, and guidelines as required.

Patient Environment

  • Keep clients in a clean environment at all times.
  • Patients area must be cleaned and organized by all field nurses.
  • Patients room should be left organized and ready for the next shift.
  • All equipment should be cleaned every day and left ready for the next shift.
  • Suction canister should be emptied and rinsed every shift. Canisters should be washed every week with warm or hot soapy water.
  • Make sure to take proper inventory on all medication and supplies at least 5 days before medication or supplies run out. Call the pharmacy for refills and inform the family so they can follow up.

We are looking for reliable LVNs who show up to work, have minimal cancellations, and perform their LVN duties proficiently and effectively. Our RN Supervisors schedule/book monthly in house visits in order to get a report on any changes that have happened within the past 30 days and supervise our field LVN's to ensure patient is receiving proper nursing care.
Benefits after 90 day probationary period:
Pay Days are every 10th and 25th of the month.
Direct Deposit. Nurse must bring a Direct Deposit form and it will take 2 pay periods for it to kick in.
Full-time Employees benefits:(Those who work 40 hrs. a week or more are considered full-time)
Sick time 40 hours per calendar year (5 – 8 hour shifts)
401K
Medical Insurance Kaiser, and AFLAC.
We offer Referral Bonuses to nurses that refer to an LVN’s and RN’s. Bonuses are issued once the new hire completes 400 hours worked.
