Description

About Us:

Avivo's Ending Homelessness division provides pathways to housing for individuals experiencing homelessness. We work with county coordinated entry systems and tribal governments and operate unique street outreach teams to connect unsheltered individuals with shelter and housing. We assist individuals in finding and maintaining housing, supporting them in scattered-site apartments and site-based housing. We provide transitional recovery housing for individuals and families participating in Avivo's treatment programs. We operate Avivo Village to address the needs of individuals experiencing unsheltered homelessness. Avivo Village is a low barrier and culturally responsive shelter with individual “homes” focused on moving residents to permanent housing. Avivo’s Ending Homelessness Division strives for the highest quality of services using culturally responsive approaches and a deep commitment to harm reduction and housing first through trauma-informed service delivery.

Your Role:

This Senior Street Outreach Navigator (Opioid) will work with individuals experiencing an Opioid Use Disorder (OUD) and long-term, chronic unsheltered homelessness. This senior level position supports the manager in assisting staff with various tasks as needed. This role specializes in implementing harm reduction best practices, distributing safer use and overdose prevention kits, connecting individuals to MAT services, and accessing SUD/OUD and mental health treatment options. You will be responsible for securing, inventorying, and tracking harm reduction supplies. This role requires organization, adaptability to an unpredictable work environment, and a willingness to work outside year-round.

Key Responsibilities Include:

  • Assist and support the manager with staff and team-related functions.
  • Train team members on assertive and progressive engagement modalities.
  • Train team members on person-centered care and language.
  • Support team members in identifying and creating healthy boundaries.
  • Train team members on Microsoft Suite and additional internal systems.
  • Train team members on de-escalation techniques and debrief with team members.
  • Serve as an expert on internal and external street outreach, harm reduction, housing, and mental health resources.
  • Proactively support team members and participate in street outreach.
    • Assist case managers in landlord connections and move-in processes.
  • Support team members in completing and uploading paperwork.

Qualifications


What You Offer:

  • High school diploma required.
  • Two or more years of experience working with single adults experiencing homelessness or similar experience.
  • Two or more years of experience supplying harm reduction services to active drug users.
  • Lived experience of homelessness and/or SUD recovery is valued.

PREFERRED:

  • Street outreach experience with unsheltered individuals.
  • Experience using HMIS.

Required Knowledge, Skills, and Abilities:

    • Expert knowledge of harm reduction, street outreach, mental health, chemical health, community resources, trauma, institutional racism, gender issues, tenant’s rights.
    • Ability to practice assertive and progressive engagement modalities.
    • Practice person-centered care and language in all interactions.
    • Capacity to take on additional responsibilities.
    • Strong communication skills (written and verbal).
    • Knowledge of community resources or where to find them.
    • Ability to proactively advocate.
    • Creative problem solver with a flexible mindset.
