AW Health Care is Hiring a Field Home Care Coordinator, Full Time, St Charles County+ Near Fallon, MO

DescriptionAW Health Care's Field Care Coordinator is a full-time professional that assumes responsibility for assisting with on-the-job training and supervision of CNAs and Caregivers, conducts openings, back up for scheduling and 40-50% covering shifts as a backup. Our Field Care Coordinator will need to have strong administrative skills and have the experience to handle all related care management activities to ensure overall satisfaction of an assigned group of clients and caregivers within the Home Care Department.

FCC Responsibilities
  • Evaluate and train staff in the field to ensure competency of the employees for Private Duty and Medicaid department;
  • Assist with in-house Caregiver training (i.e.: General Orientation, Skills Lab, Annual In-Services, On-the-Job Training, as needed.);
  • On-going education, supervision, and training of the Caregivers in the field;
  • Identifies high risk factors, barriers to goals, and strengths that will assist the client and caregivers in achieving goals;
  • Assist with the assignment and scheduling of field staff to provide care and services for Private Duty and Medicaid clients;
  • Assures quality of services by gathering information from direct observation, telephone, or in writing, all resources involved in the client plan of care;
  • Attends case management meetings as scheduled;
  • Acts as a positive role model for the company when dealing with employees and the public;
  • Communicates to Supervisor any conflicts or difficulties with employee assignment;
  • Communicates to Supervisor or Case Management RN any changes in the client’s condition;
  • Answers all pages, telephone calls or communications promptly and courteously;
  • Will rotate on-call responsibilities with other members of management;
  • Work with all members of the private pay team and across all branches (University City, Collinsville, and Wentzville) to grow private pay revenue and profit;
  • Support for on-call staff, evenings and weekends, as needed;
  • Cover shifts, in the event of staffing shortage, with direction of management;
  • Assist with office-related tasks, as well as ensuring accurate opening packets are available with an adequate supply for each week;
  • Make charts and keep filing up to date for all clients;
  • Maintain confidentiality of all information in accordance with state/federal regulations, company policy and HIPPA requirements;
  • Complete all required documentation and turn it in to the office as directed;
  • 20. Follow all policies and procedures, as they are defined in the Employee Handbook;
  • 21. All other duties assigned.
Working Environment: Works within the agency offices and patient homes and independent living communities and will be required to travel between offices and client homes.
RequirementsFCC Qualifications:
  • Must be at least 18 years of age.
  • Must have current Driver's License, your own reliable transportation and full coverage auto insurance.
  • CNA preferred, but not required.
  • Must be able to read, write and document and follow directions.
  • Must have strong background in administrative duties, some scheduling knowledge preferred and training experience.
  • Must have at least two years paid work experience as a CNA or Caregiver.
  • Must be able to work with clients that have pets or smoke.
  • Must be flexible with time and ability to adapt to changing priorities in the day.
  • Must be able to handle all types of client personalities and knowledge to handle all types of care.
  • Must have experience caring for behavioral/mentally disabled clients/patients.
  • To excel in this role, it is essential to showcase a profound understanding of growth and development principles across the entire lifespan. Additionally, you should have the capacity to analyze client data, accurately interpret pertinent information, and discern their unique age-related needs. This knowledge will empower you to deliver the care required, aligning with the agency's established policies and procedures.
Physical RequirementsAbility to perform the following tasks if necessary:
  • Ability to participate in physical activity.
  • Ability to work for extended period of time while standing and being involved in physical activity.
  • Moderate lifting.
  • Ability to do extensive bending, lifting and standing on a regular basis.
