How you make a difference
The Care Coordinator is a key member of a collaborative care team that provides treatment services to patients with mental health and behavioral disorders.
They work closely with the patient's psychiatrist, Competency Intervention Specialist, and mental health providers to develop and implement treatment plans that are tailored to the patient's needs.
The role involves providing case management services, crisis intervention, counseling, and skills training to promote effective psychiatric rehabilitation.

Qualifications & Requirements
Education
Bachelor’s degree in behavioral sciences, social work or psychology; Bachelor’s degree in a non-behavioral health field of study with significant course work in behavioral science plus a case management certificate from a certified and accredited program with a professional school of behavioral science;
May substitute a Master’s degree in social work, substance abuse, education, counseling, psychology or other related field if no prior experience in mental health.
Experience
Licenses / Certifications
Will require application as an Agency Affiliated Counselor within seven (7) days of employment.
