The Community Health Worker (CHW) – HEART/MDPCP serves as the liaison for BMS MDPCP-attributed assigned group. During their job, they will collect and input data on their activities and patients and report that program data in the means required by BMS. This CHW-HEART/MDPCP will provide informal counseling, social support, and appropriate referrals. They will provide care support, follow-up, and educational materials in clinical and community settings. By identifying and addressing issues that create barriers to care for patients, they will facilitate patients’ ability to receive services. Once a patient has related to services, they will follow up with the patient in person and/or via telephone to ensure the patient was able to fully access services and have their issues addressed. They will participate in all team meetings relevant to the MDPCP/HEART program and performs other program duties as assigned. They provide evidence-based health guidance and social assistance to applicable patients and other community members.
