The Opportunity

The Baltimore Regional Housing Partnership (BRHP), which operates the Baltimore Housing Mobility Program, is a 501(c)(3) that assists over 4,000 qualified families in moving from areas of concentrated poverty and obtaining quality and affordable housing in communities with strong schools, low crime, and ample job opportunities in high opportunity areas within Baltimore City and Anne Arundel, Baltimore, Carroll, Harford, and Howard Counties.

BRHP seeks a Client Services Navigator who is committed to connecting voucher-holding clients with health-related and other social services. Duties will include conducting non-clinical assessments of clients for wellness needs, connecting families to care, and helping families identify and fill gaps in their wellness and social service needs. The Client Services Navigator will work closely with the Housing Mobility team and the Research & Innovation team.

Essential Duties and Responsibilities

The below statements are intended to describe the general nature and scope of work for the position. This is not a complete listing of all responsibilities, duties, or skills required. Duties and Responsibilities could include:

• Serving as primary housing navigator for families with an HABC, tenant-based voucher receiving services through the Healthy Children Voucher Demonstration

• Conducting comprehensive, non-clinical assessments of clients for wellness and social needs

• Helping clients to identify, access, and coordinate care services (such as services provided by health providers, behavioral specialists, social workers, and social service agencies), including monitoring outcomes of services rendered and follow-up communication with service providers

• Developing and sustaining partnerships with community-based supportive service providers and other community stakeholders

• Monitoring the use of care services, including encouraging and motivating residents to engage with providers and participate in their own care/services management

• Proactively seeking external partners for developing and arranging educational/preventative health trainings and services for clients

• Conduct workshops, orientations, and webinars about available BRHP programs/resources

• Maintaining an up-to-date resource directory with relevant service providers throughout the Baltimore region and making it available for client use

• Advocating on behalf of clients while supporting self-advocacy and participation in service planning

• Performing regular information gathering and qualitative research relevant to the client support and services outcomes

• Convening and facilitating service partner-centered meetings

• Representing BRHP’s interest in care-related initiatives such as the Health Children Voucher Demonstration

Required Knowledge & Skills

• Experience providing outreach and client services in Baltimore City in the areas of Medicaid System Navigation, Social Needs Assessments and/or Care Coordination

• Ability to communicate with and develop positive, professional relationships with clients

• Computer skills: familiarity with Microsoft Office, Zoom and able and willing to learn additional skills

• Ability to produce outreach materials and provide outreach services to residents.

• Excellent oral and written communication and organizational skills

• Attention to detail in recording case management notes

• Proven ability to work efficiently and meet deadlines

• Proven knowledge of Medicaid System Navigation

• Outgoing, flexible, resourceful, creative, energetic and empathetic

• Must have ability to flex time to accommodate weekend and evening events as needed

• Familiarity with care providers in Baltimore region (Baltimore City, Baltimore, Anne Arundel, Carroll, Hartford and Howard Counties) preferred

Qualifications

• Bachelor’s degree in Social Work, Sociology, Counseling, Psychology, Nursing, Social Science, Public Health or a closely related field, with at least 3 years of work experience providing services to a diverse client population

Preferred Qualifications

• Certification or licensure including Licensed Clinical Social Worker or Licensed Master Social Worker
