Acute Care Clinical Pharmacist Specialist Solid Organ Transplant – Liver, Kidney, and Pancreas - *$10,000.00 sign-on bonus available! As a solid organ transplant clinical pharmacist specialist (adult kidney, pancreas, and liver transplant) you will support the BUMC-P Transplant Program. You will support our abdominal transplant pharmacist service line for a total of three pharmacists. As part of the multidisciplinary team, care is provided along the continuum of transplant in both the inpatient and ambulatory settings. The position consists of day-shift position, scheduled to work Monday – Friday. This position will rotate coverage between inpatient acute transplant services and outpatient pre-and post-liver and kidney transplant amongst the pharmacist team which is embedded in the multidisciplinary transplant team including physicians, mid-level providers, nurses, dietitians, and social workers.
As a solid organ transplant clinical pharmacist specialist you will provide direct-patient care including evaluation, implementation, and monitoring for optimal medication management and will be instrumental in all phases of transplant care. You will also be involved in organizational initiatives, education opportunities, and scholarship activities including precepting of PGY1 and PGY2 pharmacy residents. Responsibilities related to these activities may include development and implementation of process improvement strategies and cost-saving initiatives, design of new clinical pharmacy programs, development and maintenance of medication-related policies and procedures, participation in health-system and institutional committees, delivery of formal and informal educational activities to healthcare professionals, and completion of medication use evaluations and research. A PGY2 transplant residency or equivalent experience or PGY1 pharmacy practice residency with 1-2 years minimum of applicable experience strongly preferred.

Acute Clinical Staff Pharmacist As an Acute Clinical Staff Pharmacist, you will have the opportunity to provide comprehensive, collaborative, and progressive pharmacy services to our diverse patient population in both acute and ambulatory care settings.
