In this data intensive Patient Reported Outcomes Data Coordinator role you will be responsible for leading and monitoring eligible patient reported outcomes. You will identify, conduct, and track patient outreach pre and post surgery through communication via direct phone calls with patients.

This is a Monday through Friday remote role and requires candidates be bilingual in both English and Spanish.

POSITION SUMMARY

This position conducts data management, patient follow-up, reporting and other support functions related to the reporting and maintenance of patient-reported outcomes data in accordance with established guidelines set forth by the Centers for Medicare and Medicaid (CMS) for specialty-focused programs. This work includes reviewing, preparing, and assisting with collection of patient-reported outcomes by facilitating patient contact via phone or email and abstraction of clinical documentation from medical record databases.
CORE FUNCTIONS
1.
Tracks timeline for patient outreach, conducts patient calls and/or sends emails to collect patient-reported outcomes.

2. Facilitates and monitors completion of patient-reported data sets requiring application of general clinical knowledge/understanding of medical records. Compiles reports for facility and system leadership outlining compliance of CMS targets.

3. Supports monitoring, capturing, and reporting in a timely and accurate fashion for patient-reported outcomes. This includes conducting intake calls with patient pre- and post-surgery, reconciling completed intake questionnaires, and monitoring completion rates for hospital targets.
4. Demonstrate excellent customer service skills when engaging with internal and external customers, including patients, leaders, and staff members.

5. Performs analysis of customer data and reporting needs, make appropriate decisions, and provides data. Data may include, but are not limited to, electronic forms and moderate to complex reports.
6. Supports process workflow to ensure all pre- and post-surgery patient contact points are implemented until reported outcomes are collected. If additional teams are required to assist, coordination of involvement will be initiated and monitored as needed.

7. Builds and supports effective relationships with internal and external stakeholders and organizations.
8. Serves as a resource and provides facility leadership assistance to achieve optimal clinical, operational, financial, and satisfaction outcomes.

9. Organizes workload to complete assignments, working with minimal supervisionMINIMUM QUALIFICATIONS

Requires high school diploma or GED.

Requires knowledge of electronic medical record systems and medical terminology and as normally demonstrated through one to two year's experience performing clinical data review and abstraction following predefined criteria and requirements.

Bilingual in Spanish and English.

Requires exceptional interpersonal and communication skills. Must be detail oriented and have the ability to manage multiple changing priorities in an effective and organized fashion. Familiar working with patients to help complete medical forms/information. Requires strong computer skills, use of excel tools, and the ability to work with Microsoft Office products.
PREFERRED QUALIFICATIONS

Experience as medical assistant or surgery scheduler.

Additional related education and/or experience preferred.
