Assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet clients’ psychological, social, environmental, and physical needs. Provides consultation, training and support to staff as needed. Participates in in-service and other continuing education opportunities and regular supervisory sessions to review expectations and professional development. Serves as liaison with social services at BHD and patient advocacy groups in the community as needed. Other responsibilities include evaluation of service efficiency and excellence, provision of social support, crisis support, and client education. Assists in screening, third-party payor authorization, and discharge activities providing linkages to health and wellness resources across the health care continuum. Uses social services expertise and problem solving skills to achieve optimal social, psychological and resource outcomes within effective time frames. Promotes interdisciplinary collaboration with a team approach. Provides necessary counseling/therapy and discharge planning services. Participates in Risk Management. Makes indicated client referrals. Supports physicians in meeting the psychosocial needs of clients.
Master’s degree in Social Work from an accredited college or university is required. Licensure and Certification in Social Work (LCSW) in the state of Kentucky is required. EXPERIENCE: Minimum of two years in an outpatient setting or two to three years of experience in teaching and/or counseling. Computer Literacy. Excellent Communication skills, written and verbal. Demonstrates age appropriate skills and knowledge for all age groups. Excellent Customer Service and problem-solving skills. AGE APPROPRIATE SKILLS AND KNOWLEDGE: Demonstrates the knowledge and skills in addressing case management requirements/protocols, as described in departmental policies and procedure encompassing all age levels.
