Job Summary:

Provides clinical services including psychosocial assessment, individual, family and group therapies that meet the established standards of care and standards of practice as directed by supervising healthcare provider. Provides case management, community resourcing, and discharge planning services as appropriate. Assists in transitional care management as needed.

Minimum Education, Training, and Experience:

• Required Qualifications:
  1. Masters or Doctoral Degree in Social Work
  2. Active Kentucky license as a Licensed Clinical Social Worker
  3. Non-violent crisis intervention certification required after hire, not to exceed 3 months, and maintained annually. Training may be provided in-house.
  4. Must possess knowledge and skills in providing individual, group and family psychotherapy with a minimum of 1 year of clinical experience.
  5. Must show proficiency in completing assessments, treatment planning, aftercare planning and crisis management.
  6. Communication skills must be exceptional with the ability to interact professionally with professionals of multiple disciplines.
