We are currently seeking a Part Time Social Worker.

Social Worker provides medically related social services to attain or maintain the highest physical, mental and psychosocial well-being of each resident while enhancing resident’s quality of life.

Compensation: $24/hr.

Schedule: 24 hours / 3 days per week.

Essential Functions:

• Secures information to complete social history and initial assessment for new admissions and participate in discharge planning.
• Facilitate care conferences and contribute pertinent information to assist the family and staff in understanding the resident.
• Assess residents on a quarterly basis, when readmitted or when a significant change occurs and completes appropriate sections on the MDS in a timely manner.
• Complete documentation on a timely basis including assessments, discharges, readmissions, room changes, care plans, MDS documentation and any additional notes as well as maintain confidentiality.
• Prepare behavior management plans for residents as appropriate.
• Plan and conduct in-services for staff as needed.
• Attending continuing education programs designed to further social work skills with the older adult population.
• Participate in various facility committees as required as well as provide written reports as required.
• Support residents and family concerns and/or complaints.
• Provide supportive services to residents through one-to-one support and by providing community resources.
• Present social services related information to new employee orientation & resident council.
• Prepare reports for Behavioral Health Team & make referrals to the psychologist and the psychiatrist as needed and distribute associated consults to appropriate staff.
• Engage in Person- Centered Care by promoting the concepts of choice, purpose, and meaning in the life of each resident by keeping the resident at the center of all care-planning and decision making. Utilize observation and active listening to adapt to each resident’s changing needs regardless of cognitive abilities

Education and Experience

Minimum of 1-2 years of experience in long term care, assisted living, geriatric population and dementia preferred. Computer skills required. Knowledge of Microsoft Office Software and experience with MDS System preferred. Experience with Electronic Medical Records is preferred but not required.
