Job Title: Direct Support Professional Day Program

Reports To: Supervisors FLSA Status: Non-Exempt

SUMMARY: Assists consumers with developmental disabilities to achieve personal goals and to become fully engaged in their community by performing the following duties.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Guide individuals to become independent and fully engaged in their community as stated in PCP.

Encourage and accesses participation in community activities, church, social events, entertainments, recreational opportunities, etc. of the individuals’ choice.

Entertain by reading aloud, playing games, conversing, working puzzles, watching television, etc.

Encourage positive attitudes and personal awareness by discussing concerns and setting a positive example.

Encourage independence and household responsibility by engaging the individuals in cooking, cleaning, laundry, budgeting, shopping, etc.

Encourage proper grooming and personal hygiene and offers assistance as needed.

Prepare nutritious and well-balanced meals in accordance with dietary requirements.

Provide or arrange transports to activities, medical appointments, events, etc.

Follow nursing care plan, behavior plan, individual plan and other guides related to the consumer.

Administer medications and treatments according to written direction of medical professional and delegating RN.

Comply precisely with the supervision requirements stated in the PCP and behavior plans to appropriately respond, address and prevent unsafe situations that may develop quickly and unpredictably.

Provide personal care and treatments as stated in the nursing care plan.

Interact positively to redirect consumer to productive outlook.

Document services performed individual plan goals, behavior occurrences, incidents, safety concerns, home/vehicle maintenance issues, etc. in accordance with policy.

Immediately report any concerns that affect the health, safety or well-being of the individual.

SUPERVISORY RESPONSIBILITIES: None

EDUCATION and/or EXPERIENCE: High school diploma or general education degree (G.E.D.). Must successfully complete all D.D.A. required training. Adhere to specific training requirements based on the needs, preferences, and desires of the individual. Adhere to required staffing ratio and supervision level based on the PCP and the behavior plan.

LANGUAGE SKILLS: Ability to read, write and comprehend simple instructions, short correspondence and memos. Ability to effectively present information in one-on-one and small group situations to consumers, clients and other employees of the organization.

MATHEMATICAL SKILLS: Ability to add, subtract, multiply and divide in all units of measure using whole numbers, common fractions and decimals. Ability to compute rate, ratio and percent and to draw and interpret bar graphs.

REASONING ABILITY: Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

DRIVER’S LICENSE AND PROOF OF INSURANCE: A valid driver’s license with less than 4 points and proof of insurance are required for this position.

OTHER QUALIFICATIONS: This position requires reliable transportation, pre-employment and random drug screenings, criminal background check, annual TB test, communicable disease free, and travel, pre-employment, and random drug testing. Weekend and evening work are required. Current CPR and First Aid certification and bloodborne pathogen training documentation are preferred.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

While performing the duties of this job, the employee is regularly exposed to bloodborne pathogens and communicable diseases. Employee is occasionally exposed to wet and/or humid conditions, moving mechanical parts, and outside weather conditions. The noise level in the work environment is usually moderate.
