Registered Nurse Case Manager

Responsibilities:

  • Observes clients, including physical changes in body movements, speech and affect, changes in breathing, changes in mental status, and changes in behavior.
  • Ensures that all medical records are complete, accurate and understandable in accordance with Public Health guidelines. Provides additional help to clients when needed.
  • Reports any unusual or concerning developments to the Director of Nursing.
  • Patient Assessment: Perform thorough physical, psychological, emotional, and environmental assessments of patients to identify their needs and create appropriate care plans.
  • Medication Management: Administer medication, monitor its effectiveness, manage symptoms and side effects, and educate patients and their families about the medication regimen.
  • Pain Management: Implement strategies to manage patient pain and discomfort. This often involves a combination of medications, non-drug treatments, and comfort care.
  • Emotional Support: Provide emotional and psychological support to patients and their families, help them cope with the emotional aspects of terminal illness, and guide them through end-of-life decisions.
  • Care Plan Management: Develop, implement, and adjust hospice care plans in coordination with the patient, family, and interdisciplinary healthcare team.
  • Communication: Act as a liaison between the patient, family, and other healthcare providers. They need to provide clear and accurate information about the patient’s condition, treatment options, and what to expect.
  • End-of-Life Care: Assist in the management of the dying process, ensuring the patient's comfort and quality of life. This can include managing symptoms, providing hygiene assistance, and helping with activities of daily living.
  • Bereavement Support: Provide counseling and support to family members both before and after the patient's death. They can also refer families to professional grief counselors or support groups.
  • Medical Documentation: Maintain accurate and timely documentation of patient's health status, care plan, and progress notes.
  • Interdisciplinary Coordination: Collaborate with an interdisciplinary team of professionals such as doctors, social workers, spiritual advisors, and volunteers to provide comprehensive care to patients and families.
  • Education: Educate patients, families, and caregivers about the process of dying, symptom management, and self-care techniques.
  • Compliance: Ensure compliance with all federal and state regulations regarding hospice care. This includes maintaining patient confidentiality and following ethical guidelines.
  • Other duties as needed. (Note: specific duties may be assigned by the Director of Nursing.)

Skills:
- Valid RN license in Michigan.
- Minimum of 1 years of experience as a registered nurse in Home Health and Hospice.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficient in electronic medical record systems.
- Knowledge of case management principles and practices.
