New York Independent Assessor Program

Is seeking board certified Nurse Practitioners FNP, ANP, AGNP, - to support the New York statewide NYIA Independent Practitioner Panel that issues physician orders for individuals who have undergone a Community Health Assessment and are eligible for community-based long-term care services and supports. The NP will follow up with individuals who have received an assessment.

Coverage Areas needed: Western NY, Central NY, Southern Tier, North Country, Albany Capital region

• $115.00 per hour – community health in-home assessments
• Must be board certified and carry personal medical malpractice.
• Must have an active NYS Medicaid ID number.
• Must be conflict free; cannot have a Conflict of Interest (COI) - including working for an MLTC, LHCSA, Certified Home Health Agency, Insurance or ISNP
• Must have at least 2 years (3600 hours) of prior experience in any setting to qualify.
• Schedules available Monday – Friday 8am – 7pm;
• Saturdays and Sundays 10-6; Must be with NYIA-IPP 90 days for weekend eligibility
• Laptops are provided by the client.
• Training is virtual classroom, instructor-led 15 hours (paid).
• 1099 role

JOB RESPONSIBILITIES

• Responsible for reviewing and making recommendations on consumer assessments for long term care services and supports.
• The incumbent will review the completed Community Health Assessment tool in the NYS Uniform Assessment, in order to assist the Physician in the completion of a Physician’s order.
• Maintain current knowledge of regulatory changes pertaining to quality and validity of assessments.
• Assess cases presented and utilizes clinical competencies to review and validate assessment outcomes.
• Work collaboratively to monitor and address clinical quality and gaps in care

Job Types: Part-time, Contract

Pay: $115.00 per hour
