Our client is seeking experienced field registered nurses to fill openings in New York City.

This is a hybrid role - both in-person and telehealth visits.

Coverage areas needed:

• Manhattan - Will cover north of 66th Street.
• Brooklyn - Brooklyn is divided in 3 regions. RN will cover one region.
• Bronx - The Bronx is divided into 6 regions. RN will cover only one region of the Bronx
• Queens - LIC is the coverage area needed

Position Responsibilities

• Perform initial in-home evaluations of patients that includes evaluation of health status, strengths, care needs, and preferences utilizing the UAS-NY assessment tool
• Establish and maintains positive relationships with consumers, caregivers, and facility staff
• Maintain updated knowledge of the UAS-NY Assessment process
• Review consumer medical documentation and/or health referral forms, as relevant to the case
• Enter evaluation data into electronic evaluation form and transmit, as required
• Document any concerns, conflicting information, other issues that surface during the evaluation process
• Initiate communication between the Call Center and eligible consumers for enrollment into a managed care health plan

Requirements

• Active, unencumbered New York State Registered Nursing (RN) license
• Two years of field-based, visiting nurse experience
• UAS experience preferred; training is offered
• Current and valid NY driver’s license
• Reliable high-speed internet service

Pay: From $56.00 per hour
