We are looking for a Registered Nurse Case Manager role to cover the Fairmont Areas!

POSITION SUMMARY

The primary objective of the Registered Nurse (RN) Case Manager position is to oversee the management, coordination, and treatment of Baywood's clients with complex or chronic medical issues. This role plays a crucial part in guaranteeing that clients receive the best and most timely healthcare services and support.

By working closely with the office staff, healthcare teams, clients, and their families, RN Case Managers create and implement thorough care plans, arrange appointments and services, foster communication among healthcare providers, and diligently track client advancements.

ESSENTIAL FUNCTIONS

  • Performs comprehensive subjective and objective assessment of the client status that includes physical, psychosocial, and environmental parameters.
  • Ensures compliance with federal and state regulations governing home health care services by following policy and procedure consistent with agency standards.
  • Formulates individualized plan of care that incorporates analysis of assessment data.
  • Modifies and updates plan of care to reflect clients changing needs.
  • Assures thorough and accurate documentation with all clinical data but not limited to.
    • Admission through discharge, physician’s orders, skilled nursing visits and changes in conditions
  • Consistently demonstrates competency with technical nursing skills according to the legal scope of practice.
  • Maintains data collection, recording and reporting systems to ensure proper service, data collection and measurement of outcomes.
  • Participates in in-service education programs as required by state law
  • May assist with quarterly and annually clinical record reviews
  • Ensures the maintenance and confidentiality of client records.
  • Submits documentation timely and per agency policy.
  • Available for on-call shifts as assigned
  • Plans, directs, and participates in delivery of professional services to home care clients.
  • Attends staff meetings.
  • Focuses services toward diagnosis-specific client care needs.
  • Seeks consultant expertise with the President/CEO as needed to ensure agency growth.
  • Management approves time off, which is presented by RN in advance via the timekeeping systems.
  • Is flexible and works cooperatively for agency needs.
  • Assist office staff with special projects as requested and performs all other duties as assigned.

MARKETING

  • Is willing to participate in the marketing efforts of the office and does so as assigned.
  • Participates in community relations' activities as directed by management and business development/marketing positions.
  • Promotes the agency through positive representation and communication of its services.
  • Participates in committees and community relations as requested by the supervising nurse.
  • Participates in flu clinics, job/recruitment fairs, health fairs, etc. as directed.

COORDINATION OF SERVICES

  • Completes initial nursing assessments as designated by management and business development/marketing prior to the implementation of services to determine if the client meets admission criteria and assists in the assignment of qualified personnel.
  • Develops a plan of care for implementation by utilizing the nursing process and updating as necessary.
  • Assigns and coordinates services provided.
  • Performs on-site supervisory and teaching visits for the evaluation of the Home Health Aide staff on assigned cases and provides follow-up information to the management.
  • Prepares clinical progress notes and reviews documentation on a continual basis to ensure compliance with documentation standards.
  • Monitors clinical records for complete and accurate documentation and adherence to policy.
  • Provides clinical orientation to staff members on assigned home care cases.
  • Counsels the client/family in meeting nursing and related needs.
  • Ensures adequate medical supplies and equipment are provided and reordered as necessary on assigned cases.
  • Adheres to Baywood’s client care policies including the implementation of client rights and responsibilities.
  • Attends case conferences, staff meetings and in-service programs.
  • Coordinates and directs programs and staffing of personnel to promote quality services for clients.
  • Oversees all clinical services provided to clients.
  • Supervises all staff related to clinical provision of care.
  • Assures staffing of personnel to promote quality services for clients.
  • Is accountable for maintaining clinical practice in accordance with the Nurse Practice Act.
  • Inform clients and employees of the scope and nature of applicable services and community resources.
  • Responds to questions and concerns from families and clients. Promptly resolves areas of concern.
  • Establishes and implements priorities for services within the limitations of available resources.
  • Maintains liaison with attending physicians, uses community resources available for continuity of client care.
  • Responsible for maintaining open communication with care teams to assure appropriate coordinate of services.
  • Provides training or in-services to the care teams to assure the quality and appropriateness of care provided.
  • Delegates tasks according to the competencies and skills of the Home Health Aides.
  • Supervises, coaches, and disciplines team members as necessary and with consultation of management.
  • Assures that documentation for all staff is complete and appropriate to the services provided.
  • Assures that services provided are consistent with the care plan.
  • Provides leadership to the staff and assures that professional standards are in compliance with applicable state practice acts and agency policies.
  • Communicates effectively with all members of the team.
  • Demonstrates knowledge and complete understanding of internal employee manual.

FINANCIAL MAMAGEMENT

  • Understands and complies with parameters of budget relating to expenses.
  • Supports the agency's financial management efforts.
  • Secures accurate documentation for billing and payroll and assures accurate and complete records.
  • Manages cases within the appropriate financial parameters.
  • Always aware of and adheres to prior authorization time frames.
