Overview

The Value Based Care (VBC) Transformation Manager is a resource to facilitate and ensure providers and practice staff are engaged, empowered, and enabled to achieve success in a patient-centric, physician inspired and clinically driven value-based care model. This manager oversees the nurses and care team that supports the physician patient relationship inside the health system which allows for sustained engagement to address patient needs in a manner that impacts costs, quality, access, and overall patient experience. The VBC transformation team is designed to support change and what matters to physicians, providers, and the care team through a patient centered lens. The practice transformation manager supports the nurses, community health workers and activities that occur within the health system to empower and enable the entire care team to operate at top of license to improve workflows and processes, enhance point of care experience for both provider and patient, improve patient outcomes, provide gap closure, and patients to care coordination.

Responsibilities

• Ensure providers and practices are engaged, empowered, and enabled to achieve success in Value-Based Care transformation strategies
• Develop and deploy localized practice transformation strategies based on data analysis that includes measurable and time-specific goals as part of a data-informed optimization approach
• Regularly hold practice transformation meetings with the best practice team, practice leadership and key clinical influencers to monitor progress toward annual wellness completion, and discuss opportunities related to, deployed practice transformation strategies in quality
• Use internal and external data to analyze and interpret trends. Work with appropriate stakeholders to implement proactive strategies to address issues and measure the impact in quality payment programs
• Demonstrate ability to impact the quintuple aim by improving physician and provider engagement across the areas of personal, professional and lifestyle to decrease burnout by partnering to improve the health of
• Routinely assist and support AW RNs, Operations Directors, and other stakeholders in summarizing and measuring Practice Transformation Program impact, sharing best practices, provider feedback, and opportunities for organizational alignment.

Qualifications

Bachelor's degree in Nursing or healthcare related field. 4 years of healthcare management or clinic manager experience working directly with physicians and practice members with a demonstrated ability to use data, rapid cycle optimization to influence utilization, cost, and quality of care.
