Behavioral Health Connections (BHC) in Phoenix is seeking a dynamic and talented Referral Management Clinician that serves to assess and coordinate the care of patients referred for Mental Health Services. They review referral information, determine the appropriateness of services, and facilitate the connection between patients and the most appropriate facilities or treatment programs. This role is crucial in ensuring patients receive timely and effective care tailored to their specific behavioral health needs.

QualificationsEDUCATIONMaster’s degree in Social Work, Psychology, Counseling, Behavioral Health or related field.
LICENSURE/CERTIFICATIONCurrent license preferred in the State of Georgia as a Licensed Professional Counselor (“LPC”), Licensed Master Social Worker (“LMSW”), Licensed Clinical Social Worker (“LCSW”)
