Beloit Health System has an immediate Social Worker opening at Beloit Regional Hospice. In this position, the Social Worked will be responsible for Hospice Patient assessment and implementation of plan of care. The Social Worker will work provide direct Case Work Services. Required:

• Master's Degree in Social Work required
• WI/IL Social Work License is required
• Valid Driver's License
• 2 years or more of experience is preferred.
