About the Opportunity

We are seeking per diem, part time, or full time Licensed Social Worker candidates to assess hospice patients and their families to identify psychosocial, financial, environmental, and community needs. Our Hospice Social Worker also provides psychosocial support and education to hospice patients and provides families with tools to aid with coping skills. Candidate must possess a current SW License in the State of Oregon. Previous experience in hospice is a plus but is not required. We offer a competitve salary, health and voluntary insurance benefits, paid mileage, and a fun, inclusive atmosphere.

JOB SUMMARY

The Medical Social Worker provides social services to terminally ill patients and their families within a medically approved plan of care. Assists patients and families to manage problems or needs related to their psychosocial functioning and bereavement. Assists social work team with resources and interventions to provide optimal patient support.

DUTIES & RESPONSIBILITIES

1. Assists interdisciplinary team members in identifying potential high-risk bereavement factors.

2. Oversees and facilitates interdisciplinary collaboration between team members, physicians and available community resources.

3. Completes PASRR/LOC requests

4. Assists with crisis intervention, issues, concerns and ethical dilemmas.

5. Assists patient with placement issues

6. Evaluates psychosocial care provided through home visits, conferences and documentation review.

7. May provide psycho educational training opportunities to hospice team on the significance of social and emotional factors potentially faced during end of life.

8. Performs a psychosocial assessment of patient’s status and needs, including cognitive functioning, emotional status adjustment to illness and its implications, coping abilities, relationships, identity issues, unfinished business, etc.

9. Assesses the social, emotional, financial and material factors related to the patient’s illness and involves the patient and family in establishing a care plan with mutually agreed upon goals.

10. Provides crisis intervention related to patient’s terminal illness and needs for care including reducing or eliminating high risks indicators endangering the patient’s physical, mental, and emotional health.

11. Assists other hospice team members in understanding the social and emotional factors related to the patient’s illness.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)

1. Preferred minimum of one years of experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. Training will be provided for all candidates, regardless of experience.

2. Must be licensed by the Board of Licensed Social Workers in the State of Oregon

3. Maintains working knowledge of chronic illnesses, capable of explaining the major points of the illness to others.

4. Demonstrates excellent observation, problem solving, verbal and written communications; skills per competency checklist.

5. Shows ability to organize and prioritize workload independently.

6. Must be a licensed driver with an automobile that is insured in accordance with state requirements and is in good working order.

7. Self-directed and able to work with minimal supervision.

8. Successful completion of background check pursuant to ORS 181A.195.

Pay: $70,000.00 - $82,000.00 per year

Expected hours: 20 – 40 per week
