We are currently looking for an Experienced case manager to join our Team.
Must have LVN license, Office hours are Monday through Thursday 9am-5pm.

Responsibilities and Duties:

Overview and management of patient's care throughout home health service.

Coordination with all interdisciplinary members.

Continuous ongoing communication with patient and patient's primary caregivers.

Maintenance of patient's care and patient's clinical records.

Home health experience preferred.

Excellent communication and organization skills.

Other tasks as assigned.

Job Type: Full-time

Pay: $25.00 - $30.00 per hour
