Benevolent Family Services was founded in January 2014. All staff of Benevolent Family Services is devoted to professional and quality services in a timely fashion. At Benevolent Family Services our mission is to demonstrate goodwill toward those with significant mental, behavioral, and emotional health disabilities, through progressive community support. We provide mental health services from Williamsburg to Virginia Beach and the surrounding areas.
JOB TITLE: Substance Abuse Life Coach (QMHP A and/or C or CSAC)
IMMEDIATE SUPERVISOR: Clinical Supervisor, LMHP Type, Field Supervisor, Team Lead
DUTIES AND RESPONSIBILITIES:
The QMHP is responsible for services primarily to be provided via face-to-face involvement with the
individual, treatment providers, and other family members. Services are primarily within the group setting
although they may occur in various community settings. The Life Coach is required to assist in
developing and implementing the person-centered, recovery-based treatment plan. The activities
associated with the primary responsibility of this Qualified Mental Health Professional (QMHP) or
Certified Substance Abuse Counselor (CSAC) are:
Develop and maintain Individual Service Plans and Quarterly reports with individuals
Work with individuals to foster new or improved independent living skills
Case management functions of linking, arranging, and advocating for services and referrals
Maintain face-to-face contact with individuals and their support system
Maintain up-to-date individual records, and prepare required session notes in a timely and
accurate manner
Establish a good working relationship with other public and private agencies/services and
interface collaboratively with them
Establish healthy group settings to address underlying issues
Collaborate with the PMHNP and licensed providers to ensure the individual is able to reach their
goal.
Attend monthly staff meetings and supervision
Respond promptly to emails and telecommunication.
Ongoing assessment of treatment needs.
Work with individuals to foster the development of new or improved independent living skills
Maintain continuous contact with the identified consumer and key stakeholders (with a signed
authorization to release information) in planning and service delivery. These stakeholders might
include but are not limited to social workers, psychiatrists, court officials and police, teachers,
and other human service professionals
Educate individuals and family members regarding existing community services, and act as an
advocate in connecting them to these services
Respond to family crises on a 24-hour, seven-days-a-week basis
Maintain up-to-date individual records, and prepare required/necessary reports in a timely and
accurate manner
Attend court when necessary
Establish a good working relationship with other public and private agencies/services and
interface collaboratively with them
Job Objective:
Provide the necessary mental health treatment and support necessary to prevent
hospitalization, relapse, and gain independent living skills. Life Coaches are expected to
model through their behaviors and attitudes the values of Benevolent Family Services, LLC. as outlined in the Policy and Procedures
Manual. Services are time-limited interventions provided typically, but not solely, in the home of adults
with serious mental illness as described by regulations to address independent living skills or at
Benevolent Family Services during Substance Abuse Partial Hospitalization Program (SA PHP) in a
group setting.
Supervision:
Participate in individual and team supervision as scheduled
Academic/Work Experience:
Qualified Bachelors degree and must qualify as a Qualified Mental Health Professional (QMHP) or as a
Certified Substance Abuse Counselor (CSAC) through the Board of Counseling and maintain credentials
as directed by the Board. Experience with substance use is preferred. No other exceptions will be allowed.
Training:
Staff must be trained and certified in:
Cardiopulmonary Resuscitation (CPR), First Aid, and Management of Aggressive Behavior or
Behavioral Supports.
Must be certified as being tuberculosis-free upon hire
Must clear criminal background and CPS registry checks
At the time of hire, LCs must provide proof of drivers license and insurance. Should a QMHP receive a
moving traffic violation at any time after employment the QMHP must notify the HR Director about the
incident the next business day after receiving the violation. Further, QMHPs will be taken off the
schedule if they have any lapse in drivers licenses as the use of company vehicles is needed at times.
