Job Summary:
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our home health team. The Home Health RN will be responsible for providing high-quality, patient-centered care to individuals in their homes. This role involves assessing patient needs, developing care plans, and implementing treatments to promote optimal health and well-being.
Responsibilities and Duties:
- Conduct initial assessments and develop individualized care plans
- Administer medications, treatments, and therapies as prescribed by healthcare providers
- Monitor and evaluate patient progress, adjusting care plans as needed
- Educate patients and their families on health promotion and disease prevention
- Collaborate with other healthcare professionals to provide comprehensive care
- Document patient care activities and maintain accurate medical records
Qualifications and Skills:
- Current RN license in the state of practice
- Minimum of 1 year of nursing experience, preferably in home health or a similar setting
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work independently and prioritize tasks effectively
- Proficiency in electronic medical record systems
